At Lunaris Healthcare, our Hospital-to-Home service offers expert-led transitional care for adults and older adults recently discharged from the hospital, skilled nursing facility, or emergency department. This service is personally led by Mel Randle, AGACNP-BC—a board-certified acute care nurse practitioner with over 20 years of advanced clinical experience in intensive care units and post-operative recovery.
Mel has trained and practiced in some of Chicago’s most respected academic medical centers, where she developed a unique ability to care for patients with complex, high-risk conditions across every major organ system.

Transitional care is a critical time. Many patients leave the hospital with multiple diagnoses, new medications, and follow-up plans that are hard to manage alone. Our goal is to prevent complications, avoid unnecessary readmissions, and guide patients through a safe recovery—whether they’re recovering from surgery, managing a newly diagnosed illness, or waiting to see a specialist.

Mel’s training in critical care, trauma, stroke, and post-surgical recovery allows her to assess the whole person, not just isolated symptoms. Through a 1-hour telehealth visit—and optional in-home nursing support if needed—she provides medication reconciliation, symptom evaluation, care coordination, and early intervention when warning signs appear. She also ensures that patients and their families understand how lifestyle, nutrition, and medications affect recovery.
