Collection Fee: A standard $9.99 collection fee applies per lab order (paid at the lab).

Payment: Self-pay only. FSA/HSA cards accepted. (Note: if you need an itemized receipt for FSA/HSA, a provider’s office visit may be required.)

Referral Validity: Lab orders are valid for 180 days from the date of issue.

Add-On Labs: $69 base referral + $10 for each additional lab.
