Understanding perimenopause, menopause & HRT

Perimenopause is the transition before menopause.

It can start 10 years before your period stops completely

— often in your 40s, and sometimes earlier.

During perimenopause, hormone levels do not simply “drop” in a straight line. They can fluctuate up and down. Estrogen and progesterone may shift from month to month, and ovulation may become less predictable. This is one reason symptoms can feel confusing: you may feel fine for a while, then suddenly have a month where sleep, mood, energy, weight, or periods feel very different.

Perimenopause can happen even if you are still having periods. Your cycles may become shorter, longer, heavier, lighter, skipped, closer together, or more unpredictable. Some people notice symptoms before their periods change much at all.

Common perimenopause symptoms may include hot flashes, night sweats, poor sleep, mood changes, anxiety, irritability, brain fog, fatigue, low libido, vaginal dryness, discomfort with intimacy, urinary symptoms, joint aches, headaches, heavier or irregular periods, weight changes, and feeling less like yourself.

Menopause is different from perimenopause.

Menopause is reached after 12 months in a row without a menstrual period.

After menopause, estrogen and progesterone levels are generally lower, and symptoms may continue for some people.

Postmenopause refers to the years after menopause.

Some symptoms, such as hot flashes and night sweats, may improve over time. Other concerns, such as vaginal dryness, urinary symptoms, bone health, sleep, mood, and sexual comfort, may need ongoing discussion and care.

HRT stands for hormone replacement therapy.

It is also called hormone therapy or menopausal hormone therapy. HRT may be used to treat symptoms related to hormone changes, especially hot flashes, night sweats, sleep disruption, and vaginal or urinary symptoms.

HRT is not one single treatment. It can include estrogen, progesterone, or both. If you still have a uterus, progesterone is commonly used with estrogen to help protect the uterine lining. If you do not have a uterus, estrogen may be used without progesterone when appropriate.

Hormone therapy can come in different forms, including patches, pills, gels, sprays, creams, vaginal tablets, rings, or capsules. The right option depends on your symptoms, health history, risk factors, preferences, and goals.

Many patients are nervous about HRT because they have heard mixed messages. HRT, but it is also not something you need to fear without a thoughtful discussion. For many healthy symptomatic patients who are younger than 60 or within 10 years of menopause, major menopause experts state that the benefit-risk ratio can be favorable when treatment is individualized.

A good HRT discussion should include your symptoms, menstrual history, uterus status, personal and family history, breast health history, blood clot or stroke risk, cardiovascular risk, migraines, medications, labs when appropriate, and what you want treatment to help with.
