1. Service Overview

Lunaris Health PLLC (“Lunaris,” “we,” “our”) offers a self‑directed lab‑order service that allows adult Arizona residents to request laboratory tests without a clinical visit. We create and transmit your lab requisition to the lab location you specify; you pay the laboratory separately at the time of service. No medical diagnosis, treatment, or patient‑provider relationship is established through this service. Labcorp OnDemand

 

2. Eligibility

You must be 18 years or older and reside in the United States to use this service. By submitting a request, you confirm that all information you provide is true and complete and that you are requesting tests for yourself only. Labcorp OnDemand

 

3. Your Responsibilities

1. Select the exact test name(s) and lab address by visiting LaboratoryAssist.com or another CLIA‑certified lab of your choice.

2. Enter that information accurately on the Lunaris request form.

3. Pay Lunaris $69.99 (covers up to three tests) and $10 for each additional test.

4. After Lunaris confirms your requisition has been sent, return to the lab’s website to purchase and schedule your test(s).

5. Bring lab‑required ID and payment to the collection site.

If you supply incorrect or incomplete information, your order may be unusable. Once a requisition is transmitted, all fees are non‑refundable unless the error was made by Lunaris.

 

4. No Medical Advice

Test requisitions and results are provided for informational and wellness purposes only. They are not intended to diagnose, cure, or treat any disease and are not a substitute for professional medical advice. 5. Independent Laboratories

Laboratories (e.g., Labcorp, Quest, LaboratoryAssist partners) are independent entities. Their staff are not employees, agents, or affiliates of Lunaris Health PLLC. Lunaris is not responsible for laboratory billing, specimen collection, or result accuracy. Labcorp OnDemand

 

6. Reporting & Compliance

Certain test results may be legally reportable to public‑health authorities. By using this service, you consent to any mandatory reporting by the ordering provider or the laboratory. Labcorp OnDemand

 

7. Limitation of Liability

Lunaris provides this service “as‑is” and “as‑available.” To the fullest extent permitted by law, we disclaim all warranties and are not liable for direct, indirect, incidental, or consequential damages arising from (a) your choice of tests or lab, (b) delays or errors by the lab, (c) your use or interpretation of results, or (d) incomplete or inaccurate information you provide. Labcorp OnDemandHealth Labs

 

8. Indemnification

You agree to indemnify and hold harmless Lunaris and its officers, employees, and contractors from any claims, losses, or expenses arising out of your use of this service or breach of this policy. Health Labs

 

9. Not for Emergencies

This service is not suitable for urgent or emergency medical situations. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Labcorp OnDemand

 

10. Governing Law

This policy is governed by the laws of the State of Arizona. Any disputes shall be resolved in accordance with those laws.
